Abstract
Funkcija gornjeg ili centralnog motornog neurona (GMN) odgovorna je za voljnu motoričku aktivnost čovjeka. Oštećenja GMN-a dovode do onesposobljenosti u obavljanju funkcionalnih aktivnosti. Kao posljedica oštećenja javlja se poremećaj mišićnog tonusa u obliku spastičnosti, mlohavosti ili rigidnosti, javlja se nemogućnost izvođenja voljnih pokreta, patološki refleksi, oštećenje fine motorike, atrofija mišića te oslabljena gruba motorika. Rehabilitacija bolesnika sa oštećenjem gornjeg motornog neurona usmjerena je prema vraćanju specifičnih vještina odnosno funkcionalnom oporavku bolesnika, njegovom povratku u društvo, obiteljsku i radnu sredinu. Fizioterapeut u razgovoru s liječnikom primjenjuje različite postupke fizikalne terapije. Postupci poput pozicioniranja i transfera dio su neurološke rehabilitacije pacijenata s ograničenom pokretljivošću. Pravilnim pozicioniranjem i transferima smanjuju se posljedice dugotrajnog ležanja. Kako bi se izbjegle ozljede medicinskog osoblja zbog dugotrajnog dizanja i pomicanja pacijenata, potrebno je poznavati principe dobre odnosno ispravne biomehanike tijela te upotrebu pomoćne opreme u obliku daski za transfer, remena ili mehaničkog lifta.
Abstract (english)
The function of the upper motor neuron disorders (UMN) is responsible for voluntary motor activity. Damage of UMN leads to disability in performing functional activities. As a consequence of the damage there can come to disorder of muscle tone in the form of spasticity, laxity or rigidity. There is a failure to execute voluntary movements, abnormal reflexes, impaired fine motor skills, muscle atrophy and impaired motor abilities. Rehabilitation of patients with upper motor neuron damage is aimed at restoring the specific skills and functional recovery of the patient, his return to society, family and work environment. The physical therapist in a coperation with the doctor applies different procedures of physical therapy. Actions such as positioning and transfer are part of the neurological rehabilitation of patients with limited mobility. Proper positioning and transfers reduce the long-term consequences of lying. In order to avoid injury because of long-term lifting and moving of patients, medical staff need to know the principles of good and proper body biomechanics and the use of auxiliary equipment in the form of boards for transfer, belt or mechanical lifts.
