The transitional phase of DMD occurs between the ages of 6 and 11. In this phase a progressive loss of muscle strength is significant. After 8 years of life, the muscles of the lower extremities become much weaker. Walking is becoming increasingly difficult, and because of the weariness wheelchairs are sometimes needed. The head and shoulders are tilted in an arc backwards, the pelvis is moved forward, the abdomen is protruding, and the lumbar lordosis is increased. Weakness of the muscles of the lower extremities is mostly reflected in the difficult movement and the "rooster's gait" is present. When getting up, the Gowers sign is noticed, and during the walk, the patient occupies a wide base of support, staggers and leans on his fingers. The main goals of kinesitherapy in the transitional phas are: maintaining or slowing down the loss of muscle strength and elasticity, reducing or slowing the progression of contractures, maintaining or promoting mobility, alleviating or preventing reduced functional capacity, maintaining patient independence in daily activities, educating the patient and his family and improving the quality of social life. In transitional phase, it is recommended to perform exercises for maintaining or improving muscle elasticity and range of motion in the joints and functional abilities. Also, strengthening exercises, stretching exercises and exercises to increase the range of motion in the joints are recommended. Orthoses and orthopedic aids need to be applied, standing exercises and exercises to improve coordination and balance, breathing exercises and exercises to prevent the progression of scoliosis are performed.
