Abstract
Kako su bol, šepanje i ograničena pokretljivost zgloba kuka prisutni u brojnim bolestima, nije lako postaviti dijagnozu Legg-Calve-Perthesove bolesti. Nužno je dobro poznavanje specifičnih simptoma i znakova razlikovanja. Oni se mogu uočiti u pažljivo uzetoj anamnezi, kliničkom pregledu i nalazima laboratorijskih, rendgenskih i drugih pretraga. Bolest prolazi kroz četiri faze: ishemičnu, fazu fragmentacije, reosifikacije i fazu izlječenja. Liječenje se provodi neoperativnim i operativnim tretmanima radi uspostave normalnog opsega pokreta zgloba kuka, smanjenja ili izbjegavanja opterećenja glave femura koja je zahvaćena avaskularnom nekrozom s ciljem ponovne revaskularizacije glave femura. Oba tretmana liječenja iniciraju rehabilitaciju. Glavni ciljevi fizioterapijskih postupaka u liječenju su smanjiti bol, povećati opseg pokreta, povećati snagu, poboljšati ravnotežu i poboljšati učinkovitost hodanja.
Abstract (english)
Considering that pain, limping, and limited hip joint movement are present in numerous diseases, it is not easy to diagnose the Legg-Calve-Perthes disease. It is necessary to really know the specific symptoms and distinguishing signs. They can be noticed in a very carefully taken anamnesis/medical record, clinical examination and findings, laboratory, radiograph, and other medical checkups. The disease goes through four stages: ischemic, fragmentation stage, re-ossification, and curing stage. The treatment is conducted by operable and non-operable treatments to maintain normal hip joint range, flattening/decreasing or avoiding femoral head pressure, which is affected by avascular necrosis, with a goal to repeat the revascularization of the femoral head. Both treatments initiate rehabilitation. The main goals of the physiotherapeutic procedures used as treatment are decreasing pain, increasing mobility and the range of motion, increasing strength, improving balance, and effective walking.
