Abstract
Giht je jedan od najčešćih tipova upalnih artritisa, karakteriziran povišenjem razine mokraćne kiseline u serumu i taloženjem mononatrijskih uratnih kristala u zglobovima i oko njih. Giht može biti nasljedan ili češće, stečen. Razlikujemo četiri klinička stupnja gihta, to su asimptomatska hiperuricemija, akutni giht, razdoblje remisije između dva napada, te kronični giht s pojavom tofa. Strategije liječenja mogu uključivati nesteroidne protuupalne lijekove (NSAID), steroide, kolcihin, promjene u načinu života i prehrambene navike, poboljšavanje simptoma upale. Pretilost, prekomjerni unos crvenog mesa i alkoholnih pića prepoznati su kao uzročni čimbenici gihta još iz antike. Redovita aerobna tjelovježba, unos hrane siromašne purinima i vitamina C vrlo su važni za prevenciju gihta. Vježbama se održava funkcija zglobova te se spriječavaju deformacije. Edukaciju kao sastavni dio liječenja provodi liječnik, fizioterapeut, medicinska sestra, dijetetičar i radni terapeut.
Abstract (english)
Gout is one of the most common types of inflammatory arthritis, characterized by elevated serum uric acid levels and deposition of monosodium urate crystals in and around the joints. The gout can be hereditary or more often, gotten. We distinguish four clinical stages of gout, such as asymptomatic hyperuricemia, acute gout, period of remission between two attacks, and chronic gout with the appearance of the thopus. Treatment strategies may include non-steroidal anti-inflammatory drugs (NSAIDs), steroids, colchicines, changes in lifestyle and eating habits, and improving inflammation symptoms. Obesity, excessive intake of red meat and alcoholic beverages have been identified as causative agents of gout since antiquity. Regular aerobic exercises, low-purine food intake and vitamin C are very important for gout prevention. Exercises maintain the function of joints and prevent deformation. The education as an integral part of the treatment is performed by a doctor, physiotherapist, nurse, dietician and occupational therapist.
