Abstract
Tortikolis ili krivi vrat je naziv koji opisuje stanje u kojem je mišić sternocleidomastoideus skraćen na zahvaćenoj strani što dovodi do naginjanja glave na stranu na kojoj je mišić skraćen i rotacije glave prema zdravoj strani. Uzroci nastanka tortikolisa mogu biti multifaktorski (nasljedna etiologija, traume pri rođenju), ali većina autora smatra da uzrok nije poznat. Kod prirođenog tortikolisa nalaze se kontraktura i oteklina mišića sternokleidomastoideusa te su odsutni bilo kakvi koštani, upalni, neurološki i očni poremećaji. U prvim tjednima nakon rođenja dijete drži glavu nagnutu na jednu stranu i rotiranu na suprotnu stranu, tj. ako je glava nagnuta na desnu stranu rotirana je na lijevu stranu i ograničena je pokretljivost glave odnosno rotacije na bolesnu stranu. Stečeni tortikolis može nastati kao posljedica niza različitih bolesti i patoloških stanja kao što su upale, ozljede, poremećaji vida i sluha, te neka neurološka i psihička stanja. Dijagnoza tortikolisa se temelji prije svega na anamnezi dobivenoj od roditelja, kliničkom pregledu, te dodatnim dijagnostičkim pretragama. Liječenje tortikolisa obuhvaća konzervativno i operacijsko liječenje te edukaciju roditelja. Edukacija roditelja temelji se na savjetima o nošenju, hranjenju i igranju djeteta kako bi se potaknulo istezanje skraćenog mišića sternocleidomastoideusa te jačanje kontralateralnog mišića. Vrlo je važno na vrijeme dijagnosticirati tortikolis kako bi se konzervativnim liječenjem postigao oporavak te kako bi se izbjegao operativni zahvat.
Abstract (english)
Torticollis or crooked neck is a name describing a condition in which the sternocleidomastoid muscle is shortened on the impacted side, which leads to leaning the head on that side and the rotation of the head to the opposite side. The causes of torticollis can be multifactorial (hereditory, trauma during birth), but most authors believe that the cause is unknown. In the case of congenital torticollis the clinical image consists of contracture and swelling of sternocleidomastoid muscle, but there are no skeletal, inflammatory, neurological or ophthalmology disorders. Acquired torticollis can develop as a cosequence of different diseases or pathological conditions such as inflamations, injuries, sight and hearing disorders, or some neurological or psychological conditions. Diagnosis of torticollis is primarily based on anamnesis acquired from the parents, clinical examination and additional diagnostical tests.The treatment of torticollis includes conservative and operational treatment and education of parents. The education of parents is based on advice about the ways for carrying, feeding and playing with the child to encourage the stretching of the shortened muscle and strengthening of the contralateral muscle. It is very important to set the diagnosis of torticollis on time to enable the recovery with conservative treatment and avoid surgery.
